AUDITS & SAFETY
ASSESSMENTS

DEVELOPING OUR CLIENT’S CONFIDENCE
We support organisations operating in safety sensitive industries to prepare for safety audits and to be confident before and when the audits are performed. In addition, we conduct internal and external audits and safety assessments.

CAPABILITY
We provide audits and implementation support services in relation to multiple regulatory and industry standards. We conduct independent examinations of operational safety matters based on customised checklists. We cover the likes of safety policy, roles and responsibilities, safety assessment practices, training, documentation, management of change, reporting practices, and emergency response plan.

OUR ROLE
As an independent party, we test and observe an organisation’s operations and management systems and validate the implementation standard of safety practices. To conduct our evaluations, we gather information from interviews with employees and managers, reviews of documentation, and visual indicators of safety promotion.

The findings of our audits and safety assessments are included in a report which provides an objective presentation of the results of the audit or assessment.

We help with the conduct of safety risk assessment activities and the development of mitigations. We focus on the system design enhancements that are needed to improve safety performance. Effective risk management activities are complemented by support in insurance contract verification to offer ever improved protection.

SAFETY CULTURE
We support the development of a generative safety culture by training our clients’ personnel on a wide array of topics that include human factors (HF), crew resource management (CRM) and its multiple derivations, threat & error management (TEM), fatigue risk management (FRM), and line operations safety assessment (LOSA).

MONITORING

MANEGING SAFETY PERFORMANCE
Safety leading indicators express the level of safety performance achieved in an organisation and allow to assess operational safety performance through ongoing monitoring.

OUR TASK
We support our clients to manage safety performance by identifying the safety leading indicators and benchmarking against these indicators over time. The goal is to improve safety performance by measuring it and allowing for a reduction of costs and increased organisational confidence.

WHAT WE DO
We conduct an extensive review of an organisation’s safety hazards and together with our clients we identify leading indicators that are relevant, useful, and aligned with the safety objectives and targets.

MONITORING & UPDATING
We support our clients to monitor safety performance based on the identified leading indicators. We also check the ongoing relevance of the indicators to the operations, we help update the indicators to address changing conditions such as new services and updated procedures, and we support with the removal of the indicators when they are no longer relevant.

INVESTIGATING WITHIN SMS
In the past, the focus of safety investigations was often placed only on the technical causes and the immediate triggering factors, such as pilot errors. The establishment of SMS in more recent years has led safety investigations to consider multiple aspects of SMS in the causal chain of accidents and safety events.

THOROUGH UNDERSTANDING
We conduct investigations of safety events on behalf of our customers. The focus of the investigation is not placed just on the immediate causes, but in particular on the wider systemic and organisational factors so that more comprehensive fixes may be identified and implemented.

INDEPENDENT ANALYSIS
As an independent party conducting an internal safety investigation, we identify the organisational, industry and regulatory hazards in order to derive a more thorough understanding of causation and a broader sense of organisational safety accountability. Our investigations are also complemented by cost-benefit analyses to prioritise the corrective actions post-event.
